PCP Registration and Onboarding Module

The RAC-LBP Registration and Onboarding Module provides an overview of referral criteria, the RAC-LBP Model of Care as well as the crucial role primary care plays in managing low back pain patients. The module takes approximately 5-10 minutes to complete. Completion of this module is required in order to refer patients. 

NOTE: To refer into the RAC-LBP program, you must be a primary care provider (Family Medicine Physician, Sports Medicine Physician or Nurse Practitioner in a family practice) in Ontario. Please only complete this module if you meet this criteria.



To access the module, click the PCP Onboarding button below. Your RAC-LBP referral privileges will be activated within 2-3 business days of onboarding module completion.
